If you need to report an incident that is likely to lead to a claim being made against you by a customer or member of the public, please complete the following form.  

 

We need every field to be completed.  Please be sure to obtain all the information before completing the form.

Once the form is completed you will receive an email to confirm receipt and to advise you what to do next.
